- Picture ID and a utility bill with the patient’s name is required to qualify for services using CHP+. Not required for patients using Medicaid.
- Co-pay or deposit due at the time of service.

Low-cost health insurance program for uninsured Colorado children age 18 and under whose families earn or own too much to qualify for Medicaid but cannot afford private insurance.
- Children 18 and under and pregnant women 19 and over who are NOT eligible for Medicaid & do not have other health insurance.
- Fees depend on family size and income. Applicants with household income 260% of the Federal Poverty Level.
- Call to get an application, or download from the website.
- Covers primary care, emergency/urgent care, hospital services, dental care (children only), prescriptions, immunizations, maternity care and mental/behavioral health care.
- Dental provided by Delta Dental of Colorado.
- Services include preventive and diagnostic, basic restorative, oral surgery and endodontic care.
- Dental care for children services up to $1000 per year covers exams and cleanings, fillings, routine extractions and some root canal therapies. As with all CHP+ benefits, higher income families may be required to pay a small fee when they receive services.

Medicare doesn’t cover routine dental care or most dental procedures such as cleanings, fillings, tooth extractions, dentures, dental plates or other dental devices. Medicare Part A will pay for certain dental services when you’re in a hospital if you need to have emergency or complicated dental procedures. There are several plans under Medicare, so it’s best for callers to inquire about their specific benefits by calling their Medicare office.

We make every attempt to keep our list as up-to-date as possible, however, be aware that emergency and appointment availability is subject to change. We recommend that individuals contact the programs directly and we encourage them to follow any specified acceptance protocols (i.e. filling out the application, getting their name on the waiting list, etc).
